1. 32 pages valid 5 years is N26, 000
2. 64 pages valid 5 years is N35, 000
3. 64 pages valid 10 years is N70,000

Application for age 16 and above
1. Birth certificate OR age declaration. (Original and photocopy)
2. Two recent passport sized photographs in white background.
3. Guarantor’s form sworn to before a commissioner of Oaths OR Magistrate OR High Court Judge
4. Driving License OR National Identity Card (Original and photocopy)
5. Local government state of origin (Original and photocopy)
6. Passport Application Form (print out after the payment)
7. Your payment slip (print out)
8. Acknowledgement form print out)
Additional Documentation for marriage woman:
9. Marriage certificate where applicable
Additional Document for Age below 16 years
10. Parent National ID CARD
11. Child National ID CARD
12. To passport Photographs
13. Court order and letter of approval from state ministry of women and development will be required (if child where child was adopted)
14. Passport photograph of the minor to signed by the parent at the back
15. Birth certificate
16. Consent letter from either of the parent
